Member Rights and Responsibilities

We are committed to providing quality care in a safe, respectful, and supportive environment. All our clients are entitled to certain
rights and share in certain responsibilities that help us work together effectively.

 

Client Rights


All clients have the right to:

  • Respect and dignity – Be treated with courtesy and understanding at all times.

  • Privacy and confidentiality – Have your personal health information kept private, except when required by law.

  • Fair treatment – Receive services without discrimination of any kind.

  • Safe care – Expect services to be provided in a safe and supportive setting, free from abuse, neglect, or unnecessary restraint.

  • Clear information – Be informed about your condition, treatment options, medications, and possible outcomes in a way you understand.

  • Participation in care – Take part in planning your treatment and make decisions about your care, including the right to refuse treatment unless otherwise required by law.

  • Second opinions – Request a second opinion from another qualified provider if you choose.

  • Communication support – Request interpreters, alternate formats, or assistance if needed to access services.

  • Access to records – Review your health records and request corrections if you believe information is inaccurate.

  • Feedback and complaints – Share concerns or file a complaint without fear of retaliation, and receive a timely response.

  • Continuity of care – Expect treatment decisions to be based on your clinical needs, not financial incentives.

  • Advance directives – Create a mental health advance directive or appoint someone to help make care decisions if you are unable.

  • Cultural respect – Receive care that is mindful of your culture, values, and preferences.

  • Emergency access – Get emergency services when and where you need them.

Client Responsibilities

 

To help us provide the best care, we ask that you:

 

  • Work with your provider to develop and follow your treatment plan.

  • Treat staff, providers, and other clients with courtesy and respect.

  • Share complete and accurate information about your health, history, and medications.

  • Ask questions if you do not understand your care or instructions.

  • Inform your provider of any changes in your medications, including prescriptions, over-the-counter products, or supplements.

  • Keep scheduled appointments or notify the clinic's main phone line and other relevant individuals such as your PO as soon as possible if you need to cancel.

  • Let your provider know if your treatment plan is not working for you or if you are having difficulties, including financial concerns.

  • Follow clinic policies and safety procedures.
